Eligibility

    You are eligible for health plan coverage if you are a full-time employee and have been working for the company for at least 60 days

    Tiers of coverage
  • Employee Only
  • Employee + Spouse (or domestic partner)
  • Employee + Child(ren)
  • Employee + Family (includes spouse and child(ren)

How to enroll

    Complete the UHC enrollment form (see related forms and documents blow) and email the completed form to human resources.


How to change your coverage

    To cancel or make a change to your health plan coverage, complete an enrollment form and mark the changes you would like to make during the annual Open Enrollment period.

    If you experience a qualifying life event—marriage, partner's job loss, disability, new baby, change in Medicaid status or insurance coverage—you can make changes to your health care benefits outside the Open Enrollment period.

    When you make a change because of a qualifying life event
  • your change must be consistent with your life event (such as adding coverage for a new spouse/domestic partner)
  • your change will be effective on the date of the qualifying event
  • you must return the Health/Dental/Vision Plan Enrollment/Change Form to human resources within 31 days of the event—or within 60 days of change in Medicaid status
  • you must provide appropriate documentation (e.g., a marriage license or birth certificate)

Rates

    The health plans rates are based on age. COC Consulting contributes 50% of employee only monthly costs. Please refer to the rate table below for specific monthly rates based on age.

    Monthly premiums are deducted from each paycheck on a bi-weely basis.
